Architectural Registration Boards is accepted.

e.  Certification from the state or U.S. territory of original licensure and the state in which the applicant predominantly       practices and is currently licensed at time of licensure; examination(s) taken and scores received; and any disciplinary action taken or pending.

f.  A complete work history.
